RN Case Manager Per Diem - (Temple University Hospital)

About the role

The Case Manager collaborates with the patient/family, physicians, and the interdisciplinary team to facilitate the patient's progress through the acute episode of care in an efficient and cost-effective manner.

Responsibilities

• Makes evidence-based decisions and initiates appropriate protocols and order sets according to the patient's admission diagnosis.
• Influences the progression of care and assists the patient in navigating through the acute care episode.
• Presents an appropriate transition plan within 48 hours of admission.
• Serves as an advocate for the patient and family throughout the entire acute episode of care.

Requirements

• Bachelor's Degree in Nursing (BSN) required or combination of relevant education and experience may be considered in lieu of degree with commitment to obtain BSN within 2 years from date of hire.
• One year experience in acute care Case Management setting or Utilization Management at an acute care facility and/or insurance company in either UR or CM.
• General experience with knowledge of theories, principles, and concepts related to nursing practice and strong leadership capabilities.
• Knowledge of InterQual Criteria preferred.

Qualifications

• PA Registered Nurse License required or Multi-State Compact RN License required.
