RN Case Manager - Patient Flow Center - Full-time Days/Evenings

About the role

We handle multi-disciplinary communications and facilitations of care throughout a patient’s hospitalization. This includes reviewing appropriate utilization of resources, balancing with patient self-determination, recommending a level of care, confirming care orders, and communicating with various stakeholders. You'll also collaborate with the care team to develop and implement discharge plans based on individual patient needs.

Responsibilities

  • Performs pre-admission, admission, and continued stay reviews as directed by CMS guidelines, commercial payer guidelines, and hospital policy.
  • Supports service excellence, teamwork, and integrity, and maintains minimum standards for licensure.
  • Maintains knowledge of conditions of participation and coordinates and implements discharge plans.
  • Maintains open communication with ED physicians, hospitalists, attending physicians, and physician advisors.
  • Adapts to specific patient populations, ensuring privacy and clear communication.
  • Performs other related duties as required.

Requirements

  • Graduate of an accredited school of nursing.
  • Bachelor’s degree in nursing preferred.
  • No prior hospital-based experience required.
  • 3 years of hospital-based experience in discharge planning, case management, or utilization review preferred.
  • Current registered nurse license in state of practice.
  • Basic Life Support (BLS) certification from the American Heart Association.
  • Certification in Case Management (CCM) or Accredited Case Manager (ACM) preferred.
