Summary

As an important member of the multi-disciplinary care team, the Case Manager acts as a patient advocate by coordinating the care and service of patient populations across the continuum of care; promotes effective utilization and monitoring of health care resources by collaborating with the health care team; maintains accurate knowledge of federal, state and payer standards; and assumes a leadership role with the interdisciplinary team to achieve optimal clinical and resource outcomes.

Position Highlights

  • Coordinates the care and service of selected patient populations across the continuum of care
  • Promote effective utilization and monitoring of health care resources
  • Maintain accurate knowledge base of payer information for team collaboration
  • Absorb a leadership role with the interdisciplinary team to achieve optimal clinical and resource outcomes
  • Represent department in meetings with senior administration
  • Provide enhanced expert services to all departments with the organization
  • Demonstrate ongoing leadership within the hospital, community and professional organizations as required

The Ideal Candidate

  • Bachelor of Science in Nursing (BSN)
  • Minimum 5 years related experience in health care
  • Recent case manager experience
  • Demonstrated experience in oncology nursing, utilization review, quality management, and/or case management
  • Computer literate
  • Knowledge of cost accounting and financial data analysis
  • Medicare and Medicaid regulations
  • Third party reimbursement
  • Medical record coding and pricing of services

Responsibilities

  • Discharge planning for inpatients
  • Utilization Review for inpatients

Pay

Salary Range: $75,153.00 - $114,701.60
Salary ranges posted for this position represent the expected base pay range for the role. Actual compensation may vary based on location and a variety of job-related factors, including experience, skills, education, and internal equity among Team Members in similar positions.
