RN Case Manager NE- Care Coordination- Per Diem

The Care Coordination Department at the Elliot Hospital takes great pride in serving their community, patients, and families. Our Nurse Case Managers observe best practices and strive to attain the best outcomes possible for their patients. If you are a nurse with strong clinical experience/skills and you love to care for patients, but are seeking a change, please consider Case Management! We will do on the job training for experienced Nurses who meet the requirements below!

Responsibilities

• Assesses and identifies patients’ needs during the discharge planning process
• Provide resources and community referrals for continuity of care
• Care coordination with families, providers, and clinical team
• Follows Medicare conditions when needed
• Utilization Reviews

Requirements

• Bachelor's degree in a healthcare-related field or equivalent experience
• Current BSN or commitment to achieving the degree in a specified amount of time
• 3 – 5 years of case management-related health care experience.
• Certified Case Manager (CCM) or Certified Advanced Case Manager (ACCM) preferred
• Excellent clinical documentation, and successful usage of EMR systems preferred
