RN Case Manager, MAT

About the role

The RN Case Manager, MAT assesses and monitors the client's physical and psychological withdrawal from mood-altering chemicals and administers medications as prescribed with the goal of ensuring the client detoxifies safely. Case management is a collaborative process which assesses, plans, implements, coordinates, monitors, and evaluates the options and services needed to meet an individual’s health needs, using communication and available resources to promote quality and cost effective outcomes.

Responsibilities

• Assesses and monitors the client's physical and psychological withdrawal from mood-altering chemicals and administers medications as prescribed.
• Ensures the client detoxifies safely.
• Provides quality patient care via the development, implementation & evaluation of individual patient care plans.
• Acts as a liaison between the patient, the family of the patient, community partners, and organization-wide within WSMC.
• Follows established professional standards of care, WSMC guideline policy and procedures.

Qualifications

• Bachelor's degree in Nursing (BSN).
• Current licensure as a Registered Nurse (RN) in the state of [State] and certification as a Certified Case Manager (CCM).
• Experience in substance abuse treatment settings preferred.
• Knowledge of medication administration and addiction treatment principles.
