Case Manager, RN
About the role
Welcome to WelbeHealth PACE, where we provide comprehensive medical care and community-based services to help seniors stay in their homes and communities. As a Case Manager, RN, you'll guide participant care as a crucial voice on the interdisciplinary team (IDT), reporting to clinical leadership.
Responsibilities
- Conduct clinic nursing assessments to determine participant needs, preferences, and goals
- Participate in IDT meetings to develop participant care plans
- Deliver and document nursing interventions as agreed upon in participants' care plans
- Maintain a safe environment, promptly respond to physician orders, and correctly administer medications
- Collaborate with the WelbeHealth PACE Physician and other members of the IDT in coordinating participant care needs
- Communicate with participants and their families to revise care plans
- Instruct participants, their families, and caregivers regarding the disease process, self-care techniques, and prevention strategies
Requirements
- Graduate of an accredited school of nursing; BSN preferred
- Unencumbered California RN License
- Current Basic Life Support (BLS)/First Aide Certification required
- Two (2) years of nursing experience in a clinical setting with a frail or elderly population preferred
- Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter; CPR and First Aid certification upon hire preferred

Pay
Compensation consists of base salary plus bonus. WelbeHealth offers a competitive total rewards package that includes a 401(k) match, comprehensive healthcare coverage, and a broad range of additional benefits. Actual compensation will be determined based on experience and relevant qualifications.
Schedule
We offer a consistent Monday through Friday schedule – no weekends and no on call.
