About the role

Frontier Hospice is seeking a full-time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the Edmond community. In this rewarding role, you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives compassionate, clinically excellent care grounded in dignity and connection.

Responsibilities

  • Respond to clinical referral information with care and expertise, addressing inquiries and requests compassionately.
  • Partner closely with the Clinical Director to assess client eligibility and suitability for hospice services.
  • Provide direct nursing care as an RN Case Manager, ensuring patient comfort and quality of life remain paramount.
  • Deliver top-notch nursing care rooted in proven principles and techniques to ease pain and bring solace.
  • Empower patients and families through education, encouraging their active participation in creating personalized care plans.
  • Participate in call duties to ensure support is available when it matters most.

Qualifications

  • Current unencumbered license as a Registered Nurse (RN).
  • Minimum one (1) year of experience as an RN in a medical-surgical or acute care setting (hospice experience a plus).
  • Computer proficiency, including experience with Electronic Medical Records (EMR).
  • Current CPR certification (if required by state).
  • Valid driver’s license, vehicle insurance, and access to dependable transportation.

Pay

$76,000 - $84,000 per year (average pay range). Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position.

Schedule

Monday - Friday, 8:00 AM - 4:30 PM.
