RN Case Manager Hospice

About the role

As a Home Health Aide, you will provide personal care services to patients in their homes, assisting with activities of daily living such as bathing, dressing, and meal preparation.

Responsibilities

• Assist patients with personal hygiene and grooming
• Help patients with medication management
• Provide companionship and emotional support
• Ensure patient safety and comfort
• Document patient care activities accurately

Requirements

• High school diploma or equivalent
• Valid driver’s license
• Pass a background check
• Ability to work flexible hours including evenings and weekends

Qualifications

• Experience in healthcare or related field preferred
• Strong communication and interpersonal skills
• Physical ability to perform tasks in various home settings

Skills

• Excellent customer service skills
• Knowledge of basic first aid and CPR
• Ability to manage time effectively
