About the role
As a Hospice RN Case Manager (RNCM), you’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. You’ll collaborate with our interdisciplinary care team to ensure every patient receives compassionate, clinically excellent care grounded in dignity and connection.

Responsibilities
- Respond to clinical referral information with expertise and compassion, addressing inquiries to showcase our commitment to exceptional service.
- Partner with the Clinical Director to assess client eligibility and suitability for hospice services.
- Provide direct nursing care, ensuring patient comfort and quality of life remain paramount.
- Apply nursing skills rooted in proven principles to ease pain and bring solace to patients.
- Educate and empower patients and families, encouraging their active participation in creating personalized care plans.
- Participate in shared call duties to ensure support is available when it matters most.
Requirements
- Current unencumbered license as a Registered Nurse (RN).
- Minimum one (1) year of RN experience in a medical-surgical or acute care setting (hospice experience a plus).
- Computer proficiency, including experience with electronic medical records (EMR).
- Current CPR certification (if required by state).
- Valid driver’s license, vehicle insurance, and access to dependable transportation.

Pay
$72,500 - $77,500 per year (average pay range). Final compensation will be determined based on relevant experience, qualifications, and role responsibilities.
Schedule
Monday - Friday, 8:00 AM - 4:30 PM.
