RN Case Manager – Hospice

About the Role

Manage a case load of 15 patients with 4-5 visits per day depending on area and acuity level of patient, complete charting same day. RN must be willing to perform all service level nursing (routine, GIP, continuous, and respite), must be willing to perform admission, SIA and death visits.

Requirements

• WA/compact RN license
• AHA BLS
• Active DL and Auto Insurance
• 1+ years minimum of current Hospice Case Management experience
• Licensed in WA and preferably OR state nursing board
• Reliable vehicle to cover a 60 mile service radius (mileage will be covered at IRS standard rate)
• Clean background check in WA and OR
• Drug screening
• Influenza/TB/Covid vaccination records
• Valid auto insurance

Preferred Qualifications

• Acute/Med Surg Experience

Schedule

• 8:30AM - 5:00PM/M-F or alternative schedule Sunday-Thursday
• 40 hours guaranteed
• Possible weekends and call

$1800.00 - $2500.00
