About the role
Assesses patients, develops and coordinates individualized hospice care plans, manages symptoms and pain, provides nursing care and education, collaborates with the interdisciplinary team, and supports patients and families to ensure comfort and quality of life during end-of-life care in the home setting.

Requirements
- VT or compact RN license
- BLS certification
- 1+ year of home health hospice case management experience
- Oasis experience
Schedule
- 16-week assignment
- Day shift, Monday–Friday with some weekend coverage
Pay
$1,800.00 – $2,700.00 per week
