RN Case Manager, Home Health - FT

About the role
Integrates health care for clients from pre-admission to post discharge through coordination and sequencing of care. Enhances patient/resident flow and interdisciplinary communication, promoting early intervention. Evaluates outcomes based on integration among established clinical, financial, and utilization data. Functions in structured and unstructured health care settings, utilizing independent nursing judgment. Applies age-specific structured care methodologies for neonatal, pediatric, adolescent, adult, and/or geriatric patients/residents. Coordinates, facilitates, and negotiates with patients/residents, family members, and the health care team. Monitors, evaluates, and trends patient/resident responses using structured care methodologies. Demonstrates fiscal responsibility by controlling variances and duplication. Requires independence, assertiveness, and critical thinking. Must document clearly, solve problems, and work with minimal direction. Incorporates teaching/learning principles and adapts teaching to age groups. Facilitates groups and implements projects as needed. Establishes priorities, demonstrates flexibility, and possesses strong organizational skills. Knowledgeable about reimbursement for services provided.
Schedule
- Full time
- 8-hour day shifts
- Weekly hours: 32
- Primarily day hours with scheduled weekends and occasional evenings
Pay
Salary range: $34.00 - $51.00 per hour
Qualifications
- Bachelor's degree in Nursing strongly preferred
- Graduate from an accredited nursing program (e.g., CCNE, ACEN, or NLN CNEA)
- Minimum of two years’ clinical experience preferred
- Unencumbered RN license with the State Board of Nursing where practicing and/or multistate licensure if in a Nurse Licensure Compact (NLC) state
- Required department-specific competencies and certifications
- Depending on location, a valid driver’s license may be required
Work environment
- May be exposed to communicable or infectious diseases, hazardous materials, and injury from performance of assigned duties
- Subject to multiple sensory and environmental stressors
