About the role

We are seeking an experienced Home Care Registered Nurse/Field Case Manager to provide skilled nursing visits and coordinate patient care in patients' homes across multiple counties.

Responsibilities

• Conducting assessments
• Developing care plans
• Educating patients and families
• Managing complex wound care, PICC lines, TPN, antibiotics, catheters, tracheostomy, and monitoring health conditions to promote independence

Qualifications

• IL RN license
• BLS certification
• College diploma in nursing
• At least 1 year of nursing experience

Skills and duties

• Performing OASIS assessments
• Documenting according to regulatory standards
• Acting as a team leader
• Supervising home health aides
• Managing physician orders
• Ensuring timely communication with healthcare providers
