RN Case Manager Home Health

Full‑time position located in the Rockwall office.

Responsibilities

• Consult with the physician (attending) to establish and alter plans of treatment.
• Participate in all quality‑improvement activities of the agency.
• Enforce personnel policies and report all personnel matters requiring action to the Director of Nurses.
• Report directly to the Director of Nurses regarding problems with patient admission or continuation of care.
• Order equipment for patients and coordinate equipment pick‑up upon discharge.
• Ensure IDT meetings are conducted with appropriate personnel.
• Participate in the after‑hours on‑call rotation, including holidays and weekends.
• Strive for self‑improvement through attendance at in‑services, workshops, conferences, and professional organizational meetings.
• Assist with in‑service programs for home health aides as needed.
• Keep all physicians’ orders current.
• Coordinate services and delegate responsibilities.
• Evaluate the quality of care given to patients and make revisions or modifications when necessary.
• Manage admission of new patients, recertifications, discharges, ROC, SCIC, transfers, and conduct home visits to problem patients as needed.
• Provide appropriate reports to designated individuals in a timely manner.
• Assist personnel in developing various skills relating to patient needs.
• Participate in staff development activities.
• Assume additional duties if indicated.
• Participate in the orientation of new staff.

Requirements

• Previous Home Health Case Manager experience is required.
