About the Role
The Registered Nurse, Home Health Case Manager, provides professional nursing services in accordance with physicians’ orders or plan of care. The RN Case Manager determines nursing problems for an assigned group of patients/residents, develops and implements care plans, and evaluates their effectiveness. This role supports the Friendship Village mission, vision, and values.
Responsibilities
- Provide professional nursing services per physicians’ orders or care plans.
- Determine nursing problems for an assigned group of patients/residents.
- Develop, initiate, or assign care plans to resolve identified problems.
- Evaluate the effectiveness of care plans in resolving problems.
- Complete training as required by policy.
- Support the organization’s mission, vision, and values.
