RN Case Manager - Full-Time Days

Role: Home Health RN Case Manager responsible for delivering comprehensive in-home care to post-acute patients. Responsibilities: Assessing patient needs, developing and coordinating personalized care plans, supervising LPNs and aides, managing patient episodes from admission to discharge, providing education to patients and caregivers, communicating with the healthcare team, ensuring regulatory compliance, and participating in quality improvement activities. Qualifications & Skills: Minimum of one year in Medical/Surgical nursing, with preferred ICU and home health experience. Strong clinical and critical care assessment skills, excellent communication, leadership, organizational, and problem-solving abilities. Must hold RN licensure, BLS certification, and valid driver’s license with insurance. Knowledge of home health operations and regulatory guidelines, including OASIS, is preferred. Additional Requirements: Ability to work independently, perform routine and complex procedures, and participate in ongoing professional development. Must meet productivity standards and be flexible for weekend and call rotations. Complies with organizational policies and standards.
