About the role

The RN Case Manager is responsible for providing comprehensive care coordination and clinical oversight to patients in the field setting. This position requires strong clinical skills, excellent communication abilities, and the ability to work independently and as part of a multidisciplinary team.

Responsibilities

• Assess patient needs and develop care plans
• Coordinate care among various healthcare providers
• Monitor patient progress and adjust care plans as necessary
• Provide education and support to patients and families
• Ensure compliance with regulatory standards and policies

Requirements

• Bachelor's degree in Nursing or related field
• Current RN license in Wisconsin
• At least 2 years of relevant clinical experience
• Strong computer skills, including proficiency with electronic health records (EHR)

Pay

The salary range for this position is $50,000 - $60,000 annually.
