The RN Case Manager is responsible for providing comprehensive care coordination and support to patients in the Stevens Point, WI area. This position requires excellent communication skills, strong organizational abilities, and a commitment to patient-centered care.

Responsibilities

• Assess patient needs and develop care plans
• Coordinate care among various healthcare providers
• Manage patient transitions between settings of care
• Provide education and support to patients and families
• Ensure compliance with regulatory standards and policies

Requirements

• Bachelor's degree in Nursing or related field
• Current RN license in Wisconsin
• At least 2 years of relevant clinical experience
• Strong interpersonal and communication skills
• Proficiency in Microsoft Office Suite
