RN Case Manager - Clinical Supervisor

About the role

Aveanna Healthcare is seeking an experienced RN Case Manager to join our clinical team in Corona, CA. This role offers the opportunity to guide and support our field staff, monitor clinical excellence, and provide autonomy in scheduling.

Responsibilities

• Promote and enhance the level of clinical expertise of staff to provide safe and quality nursing care for our patients.
• Review physicians’ orders and update the plans of care and documentation, as necessary.
• Assist with interviewing, selecting, training, and validating new clinical staff.
• Participate in the release of patients from the facility to their home through “Start of Care Assessments” and “Re-evaluations.”
• Collaborate with the operations and business development team.
• Internal case management, evaluation, and development of nursing talent.
• Ensure adherence to State, Federal, Local, and Aveanna policies and procedures.
• Quality assurance of clinical documentation.
• Care coordination with ancillary healthcare providers.

Qualifications

• Good standing license (RN) in the state in which the clinician will practice.
• At least 2 years of hands-on RN nursing experience within the last 5 years.
• Able to travel to patients’ homes in designated territory.
• Reliable transportation, valid driver’s license, and passing MVR check.
• Current CPR certification from AHA or ARC.
• Willingness to supervise nurses providing in-home skilled nursing care to infants, children, adolescents, and/or adults.
