RN Case Manager, Cardiology - Las Vegas, NV

Role Overview

Key Responsibilities

• Apply case management standards to high-need patients
• Act as a patient advocate and resource for the care team
• Develop and monitor personalized care plans with patients and providers
• Assist with remote monitoring and telehealth initiatives
• Educate patients on disease management and lifestyle modifications
• Maintain accurate records, analyze data, and report outcomes
• Career transitions and reduce readmission rates
• Perform clinical assessments and provide telephonic nursing support
• Utilize clinical software and demonstrate strong communication skills

Qualifications & Skills

• Active RN license in Nevada and BLS certification
• Minimum 3 years of critical care nursing experience
• Knowledge of case management, UM, and plan benefits
• Strong clinical assessment, critical thinking, and communication skills
• Proficiency with MS Office and practice management systems

Preferred Qualifications

• Bachelor’s degree in healthcare or related field
• CCM and ACLS certifications
• Experience in cardiology and case management

Work Environment & Compensation

Normal clinic setting with travel within the local area. Compensation ranges from $60,200 to $107,400 annually, with benefits including health coverage, incentive programs, 401k, and more.
