RN Case Manager, Cardiology - Las Vegas, NV

Role Overview

The RN - Cardiology Case Manager is responsible for care coordination of high-risk, high-need cardiology and CHF patients, both in-person and telephonically.

Key Responsibilities

• Develop individualized care plans
• Act as patient advocate
• Ensure smooth care transitions
• Monitor treatment adherence
• Provide telephonic nursing support
• Collaborate with providers
• Auxiliary with remote telemonitoring
• Promote patient self-management through education

Qualifications & Skills

• Active Nevada RN license
• 3+ years in critical care nursing
• Strong clinical assessment, critical thinking, and communication skills
• Knowledge of case management, plan benefits, and familiarity with relevant software
• Bachelor's degree (preferred)
• CCM certification (preferred)
• ACLS certification (preferred)
• Cardiology experience (preferred)

Environment & Compensation

• Clinic setting with frequent local travel
• Salary ranges from $60,200 to $107,400 plus benefits, incentives, and professional development opportunities
