About the Role
Our Case Management team is expanding! We are seeking RN Case Managers to join our team. Those with a background in Case Management or looking to explore a new opportunity in nursing are encouraged to apply.
As an RN Case Manager, you will use your clinical expertise to assess patients across the continuum of care, coordinate individualized care plans, and collaborate closely with physicians and multidisciplinary teams. You will advocate for patients, review diagnostic results and level-of-care needs, and help ensure patients receive the right care at the right time. This is a great opportunity for an RN who enjoys clinical decision-making, care coordination, and making a meaningful impact on patient outcomes.
Responsibilities
- Coordinate discharge planning to assure that the patient progresses through the continuum of care and is discharged to the least restrictive environment
- Coordinate hospital activities concerned with case management and discharge planning
- Ensure compliance with quality patient care and regulatory requirements
Requirements
- Minimum of two (2) years of nursing experience
- Graduate of an accredited nursing program
- Licensed/registered RN in New York State
Qualifications (Preferred)
- Bachelor of Nursing degree
- Experience in hospital Case Management or discharge planning
- Certification in case management (e.g., Commission for Case Management Certification (CCMC) or Association of Rehabilitation Nurses)
Schedule
Hours are assigned based on clinical and operational needs, as well as staff availability, and may vary. Typical shifts may include: Monday-Friday, 8:00am-4:00pm with occasional weekend rotation.
Pay
$40 - $56 per hour
