RN, Case Manager
- About the role:
The successful candidate will provide comprehensive case management services to patients in need.
- Responsibilities:
- Assess patient needs and develop care plans
- Coordinate with healthcare providers and other agencies
- Monitor patient progress and adjust care plans as necessary
- Ensure compliance with regulatory standards and policies
- Requirements:
- Bachelor's degree in Nursing or related field
- Current California RN license
- At least 2 years of relevant experience
- Qualifications:

- Ability to work independently and as part of a team
- Proficient in Microsoft Office and electronic health records systems
- Skills:
