RN Case Manager

About the role
The RN Case Manager is responsible for the ongoing development, implementation, and evaluation of processes within the Case Management Plan. This role involves a highly collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet the client’s/patient’s health and human service needs. The position provides advocacy, communication, collaboration, resource management, and education to interdisciplinary team members while ensuring effective allocation of hospital resources and high-quality patient care. The role also ensures compliance with regulations and policies of all review organizations related to billing and fiscal intermediaries, implementing processes to meet Federal/State regulations.
Responsibilities
- Develop, implement, and evaluate processes of the Case Management Plan.
- Assess, plan, implement, coordinate, monitor, and evaluate services required to meet patient health and human service needs.
- Provide advocacy, communication, collaboration, and education to interdisciplinary team members.
- Manage hospital resources effectively while promoting high-quality patient care.
- Ensure compliance with Federal/State regulations and billing policies of review organizations.
Requirements
- Unrestricted CO/compact RN license.
- Bachelor’s Degree in Nursing.
- 3+ years of clinical experience in a hospital setting.
- 5+ years of case management experience in a hospital setting.
Qualifications
- Certification in case management or willingness to obtain within 2 years of employment.
- Master’s degree (preferred).
Schedule
- Length of assignment: 8 weeks.
- Full-time; shift details to be discussed in interview.
- Start date: ASAP.
