About the role
Coordinates patient care across the continuum, conducts utilization review to ensure appropriate level of care and resource use, and facilitates safe, timely discharge planning by collaborating with patients, families, physicians, and the interdisciplinary healthcare team.

Requirements
- ME/compact RN license
- BLS certification
- 4-5 years RN experience
- 3+ years of acute clinical experience (not just Case Management experience)
- Prior completed travel assignments
- 2-3 prior Case Management travel assignments completed
- Experience in Care Coordination, Discharge Planning, and Utilization Review
Schedule
- 13-week assignment
- Shift: 7:30 AM – 4:00 PM
- Every third weekend: 7:30 AM – 4:00 PM
- 40 hours per week
Pay
$1,800.00 - $2,700.00 per week
