About the role

The Case Manager (CM) ensures high quality, patient-centered care by managing rehabilitative care to ensure optimum outcomes. As an active member of the rehab team, the CM provides, coordinates, and directs care specific to the needs of each Rehab patient. The CM collaborates with the Rehab Program Director and Facility Case Management Director regarding departmental functions, and coordinates efforts within the rehabilitation team. Your voice to influence patient care is valued and empowered at every turn—whether through open, collaborative relationships with your direct manager or more formal opportunities through hospital councils and national nursing initiatives. You'll help shape decisions that elevate both patient outcomes and the future of nursing.

Responsibilities

• Guide patients and families through program orientation, explaining the rehabilitation philosophy, Medicare and insurance benefits, discharge criteria, and patient rights
• Coordinate education for patients, families, and caregivers to encourage participation in treatment planning, goal discussions, and family conferences
• Develop and implement individualized treatment plans that reflect the patient’s strengths, needs, and personal recovery goals
• Complete psychosocial assessments and discharge planning evaluations, clearly documenting findings and communicating needs across the care team
• Collaborate closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-centered care

Requirements

• Registered Nurse (RN) license
• Associate Degree
• Basic Cardiac Life Support must be obtained within 30 days of employment start date
• At least 1 year of experience in Rehab
• Previous experience in case management preferred
