RN Case Manager in the Rehab Unit.

Responsibilities

• Coordinating care for adult rehabilitation patients by managing assessments, care plans, and safe discharges
• Collaborating with physicians, therapists, social workers, and families to enhance recovery outcomes
• Performing utilization reviews, documentation, and patient and family education
• Coordinating community resources to support patient needs
• Ensuring patient-centered care, optimizing recovery, and supporting safe discharges within a team-oriented environment that promotes professional growth

Qualifications

• Registered Nurse (RN) license
• Experience in patient care coordination, preferably in rehabilitation or similar settings
• Strong communication and teamwork abilities
• Knowledge of utilization review, documentation, and patient education
• Commitment to evidence-based care, integrity, and continuous learning
