Rehabilitation Case Manager

Role: Manage case coordination for patients at an inpatient rehabilitation facility. Responsibilities Conduct patient assessments and develop individualized treatment plans.Maintain thorough care documentation and provide education to patients and their families.Secure insurance authorizations and collaborate effectively with the care team. Qualifications & Skills Current Oregon licensure as an RN, LCSW, RT, PT, or SLP.Case management or rehabilitation nursing certification is preferred.Strong communication, organizational, and teamwork skills.
