Position Summary

Make a difference in people's lives while enjoying a flexible schedule. Our care management team is seeking an RN Care Manager to provide personalized care and support to caregivers and their loved ones in Augusta GA and surrounding counties (Macon, Columbus, Warner Robins, Savannah, Albany).

What You Will Do

• Coverage of home visits in assigned territory to assess home safety, gather changes in health status, and provide clinical coaching and support including review of medications
• Following an established care plan, provide ongoing coaching and support to caregivers via app, including resource referral, and reinforcement of best practices
• Develop trusting relationships with caregivers to influence change and reduce caregiver burnout
• Document and review records for accuracy and timeliness, ensuring compliance with regulatory and accreditation standards
• Perform other duties as assigned

What You Will Bring

• Must hold an active Registered Nurse (RN) License in the state of employment
• Minimum two years of recent experience in direct care of elders or adults with disabilities
• Experience in case management, care planning and assessment for cases including but not limited to eldercare, people with disabilities, and other complex medical needs in a health care setting
• Proficiency in common software and web-based applications (e.g. MS Office, case management systems)

The Pay Range

The pay range for this position is $68,700 - $84,200. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
