About the role
Network Health is seeking a RN Care Manager to join our team remotely in Wisconsin. This position is responsible for managing patient care and coordinating services.
Responsibilities
- Manage patient care coordination
- Coordinate services with other healthcare providers
- Ensure patients receive appropriate care and support
- Communicate effectively with patients and their families
Requirements
- Bachelor's degree in Nursing (BSN)
- Current RN license in Wisconsin
- At least 2 years of clinical nursing experience
- Strong communication and interpersonal skills
Qualifications
- Experience in care management or related field
- Knowledge of healthcare policies and procedures
- Proficiency in Microsoft Office Suite

Pay
