About the role
The Inpatient Social Worker role involves assessing, planning, advocating, and coordinating patient care from admission to discharge as part of an interdisciplinary team.
Responsibilities
- Ensuring safe transitions post-hospitalization
- Overseeing length of stay
- Maintaining patient safety and quality of care through performance metrics and best practices
Qualifications
- Graduate of an accredited School of Nursing
- Bachelor's Degree of Science in Nursing (minimum)
- Current licensure in the state of employment
- BLS (CPR) certification at hire or within 90 days
- 2-3 years of acute care hospital experience preferred
- Care or Utilization Management experience preferred
- Certification in Case Management is a plus
