About the role

The Registered Nurse (RN) is a professional caregiver responsible for assessing, planning, implementing, and evaluating patient care. The Case Manager RN coordinates care and services for patients needing assistance or meeting Care Management criteria, working closely with the healthcare team.

Responsibilities

  • Catalog in-depth information about a patient's medical, functional, and social conditions to identify individual needs and develop a plan to meet those needs.
  • Work with patients and their families/caregivers to establish specific goals and actions based on the assessment.
  • Coordinate discharge planning and execute specific interventions to meet established goals.
  • Organize, integrate, and coordinate necessary resources to achieve the goals and plan.
  • Assess the patient's prior level of functioning, access to and use of community resources, and available support systems.
  • Collaborate with physicians and other members of the health care team on behalf of the patient.
  • Identify appropriate admission and continued stay issues and enhance the quality of patient care through effective and efficient use of resources.
  • Attend, facilitate, and participate in rounds and case conferences.
  • Advocate for patient rights and monitor efficiency and availability of services, evaluating outcomes through variance tracking and data collection.
  • Identify needs, facilitate, or provide education to physicians and ancillary departments/nursing units regarding case management, discharge planning process, and roles.
  • Participate in multidisciplinary groups and develop guidelines.

Requirements

Graduate of an accredited school of nursing, Bachelors Degree in Nursing (BSN), Current RN license in the state of practice or compact license with the ability to obtain RN license in the state of practice, Minimum of three (3) years of experience as a Registered Nurse in an acute care setting, Preferred at least five (5) years of nursing experience, Prior knowledge of managed care / utilization review.

Qualifications

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy, Frequent interactions with patient care providers, patients, and visitors that require verbal communication, Computer use and typing for documentation, Lifting and utilizing full range of movement to transfer patients, Pushing/pulling gurneys and portable equipment, Standing in a stationary position for an extended period of time, Walking and assisting with transporting/ambulating patients and obtaining and distributing supplies and equipment.

Skills

Knowledge of managed care / utilization review, Ability to work with patient care providers, patients, and visitors, Ability to communicate effectively, Ability to work independently and as part of a team, Ability to prioritize tasks and manage time efficiently, Ability to handle stressful situations and emergencies, Ability to analyze data and make informed decisions, Ability to collaborate with physicians and other members of the healthcare team.
