What You’ll Do
- Conduct comprehensive assessments for Medicare beneficiaries, including medical, behavioral, and social needs.
- Develop and manage individualized care plans aligned with evidence-based clinical guidelines.
- Provide ongoing chronic disease management for conditions such as diabetes, heart failure, COPD, and hypertension.
- Perform medication reconciliation and support medication adherence.
- Deliver patient education, health coaching, and self-management support.
- Career coordination, documentation, and patient engagement skills.
- Coordinate care across primary care providers, specialists, hospitals, post-acute facilities, and community resources.
- Manage transitions of care following emergency department visits and hospitalizations.
- Identify and close preventive care, screening, and quality care gaps.
- Design, implement, and continuously improve care management workflows from patient enrollment through ongoing engagement.
- Develop documentation standards that support care management and value-based reimbursement programs.
- Partner with operations and analytics teams to establish caseload models, outreach strategies, performance metrics, and scalable processes.
- Evaluate and optimize care management technology and electronic health record workflows.
- Serve as a clinical thought partner to leadership on value-based care initiatives and accountable care strategies.
- Support quality improvement initiatives, including HEDIS, STARs, Transitional Care Management (TCM), and other value-based performance measures.
- Document care management activities accurately to support program requirements and reimbursement.
- Identify opportunities to reduce avoidable emergency department visits and hospital admissions.
- Collaborate closely with interdisciplinary care teams, patients, caregivers, and providers through phone and virtual communication.
- Escalate clinical concerns appropriately while supporting coordinated, patient-centered care.
What You’ll Bring
- Active Registered Nurse (RN) license in New York State.
- Three or more years of clinical nursing experience, preferably in primary care, care management, population health, or a related setting.
- Experience supporting Medicare populations strongly preferred.
- Demonstrated success building, improving, or optimizing clinical workflows.
- Familiarity with value-based care models, including ACOs, MSSP, APCM, CCM, and related programs.
- Strong care coordination, documentation, and patient engagement skills.
- Operational mindset with the ability to thrive in an evolving, early-stage environment.
- Knowledge of social determinants of health and community-based resources.
- Excellent communication and relationship-building skills.
- Able to work in a hybrid role with regular in-person collaboration in New York City.
- A builder mentality with a passion for creating scalable solutions.
- High emotional intelligence, humility, adaptability, and a collaborative approach to problem-solving.
Compensation
Salary Range: $85,000–$110,000 annually.
