RN Care Manager - Part Time, Weekend Option

About the role

The Care Manager facilitates care transition planning within an interdisciplinary healthcare team to achieve quality outcomes, reduce costs, and ensure patient safety.

Qualifications

• Graduation from an accredited School of Nursing with a Bachelor's Degree in Nursing
• Current licensure in the state of employment
• 2-3 years of experience in an acute care hospital setting
• Basic Life Support (BLS) certification at hire or within 90 days
• Physical demands: ability to lift 50 pounds, stand, walk, and perform various physical movements.

Preferred Qualifications

• Care Management or Utilization Management experience
• Certification in Case Management
• Strong communication, assessment, time management, critical thinking, and computer skills
• Knowledge of InterQual criteria (preferred)
