RN Care Manager - Part Time, Weekend Option

About the role

The Care Manager facilitates transition of care planning with an interdisciplinary healthcare team to achieve quality outcomes, reduce costs, and ensure patient safety.

Responsibilities

• Collaborate with an interdisciplinary healthcare team for care planning and transition of care.
• Ensure quality outcomes, cost reduction, and patient safety.

Requirements

• Graduate of an accredited School of Nursing.
• Bachelor's Degree of Science in Nursing.
• Current nursing license in the state of employment.
• 2-3 years of experience in an acute care hospital setting.
• BLS (CPR) certification at hire date or within 90 days of hire.
• Ability to push, pull, and/or lift 50 pounds regularly.
• Prolonged standing and walking during each shift.
• Ability to grip, reach, bend, kneel, twist, and squat to perform duties.

Preferred Qualifications

• Care Management or Utilization Management experience.
• Certification in Case Management.
• Knowledge of InterQual criteria.

Skills

• Strong verbal and written communication and facilitation skills.
• Strong assessment, time management, and critical thinking skills.
• Advanced computer skills.
• Ability to interact effectively with a variety of people and situations at all levels of the organization.
