About the role
This position is for a Registered Nurse Care Manager in the Neonatal Intensive Care Unit (NICU) at Cincinnati Children's.
Responsibilities
- Coordinate patient care across the continuum for an assigned patient population
- Collaborate with interdisciplinary team members to ensure quality outcomes and efficient utilization of resources
- Facilitate communication between patients, families, and healthcare team members
- Develop and implement care plans to meet patient and family needs
- Monitor patient progress and outcomes
- Facilitate transitions of care and discharge planning
Requirements
- Current RN licensure in the state of Ohio or compact state
- BSN required
- Minimum 2 years of recent NICU nursing experience
- BLS and NRP certification required
Qualifications
- Strong clinical assessment and critical thinking skills
- Excellent communication and interpersonal skills
- Ability to work collaboratively in a team environment
- Knowledge of care coordination and case management principles
- Experience with electronic medical records
