RN Care Manager – Hospice

Start Date: 8/24/2026 • Pay Rate: $1,800.00 - $2,700.00

About the role

The RN Care Manager provides patient care and services that require substantial, specialized bedside nursing skills for symptom management, crisis intervention, and family education. Candidates must demonstrate excellent written and verbal communication, problem-solving, and decision-making abilities, along with experience developing and maintaining rapport with patients and families and collaborating effectively within an interdisciplinary team.

Responsibilities

• Manage patient care with specialized bedside nursing skills
• Provide symptom management and crisis intervention
• Deliver family education
• Develop and maintain rapport with patients and families
• Collaborate with an interdisciplinary team

Requirements

• Active VA or compact RN license
• BLS certification
• Valid driver’s license
• Good driving record and reliable transportation
• Minimum 1 year of hospice experience

Preferred Qualifications

• Additional certifications or experience in hospice care

Schedule

• 13-week assignment
• Flexible Monday–Friday, 8:30 AM–5:00 PM (4/10 schedule considered)
