Hospice Care RN Manager

About the Role

We are seeking an experienced Patient Care Manager to oversee hospice clinical operations, ensuring high-quality, compassionate care for patients and their families.

Responsibilities

• Manage patient care services and schedule visits.
• Triage referrals and lead interdisciplinary meetings.
• Mentor staff and ensure regulatory compliance.
• Support quality improvement initiatives and operational efficiency.

Requirements

• Current RN license and graduation from an accredited nursing program.
• At least 3 years of direct patient care experience, including 2 years in hospice or home care.
• Knowledge of hospice principles, industry standards, and compliance regulations (Medicare, Medicaid, JCAHO, ACHC).
• Strong leadership, interpersonal, organizational, and clinical documentation skills.

Preferred Skills

• Bachelor of Science in Nursing (BSN).
• Leadership or supervisory experience in hospice or home health.
• Knowledge of EMR systems and telehealth platforms.
• Certification in Hospice and Palliative Nursing (CHPN).
