About the role
We are seeking a compassionate and dedicated RN Care Manager to provide skilled nursing care to patients in their homes.
Responsibilities
- Conduct initial and ongoing comprehensive patient assessments
- Develop, implement, and update individualized plans of care
- Manage an assigned patient caseload efficiently
- Cookordinate patient care with physicians and interdisciplinary healthcare teams
- Provide skilled nursing care in the patient's home
- educate patients and caregivers regarding treatment plans and disease management
- Maintain accurate and timely clinical documentation
- Monitor patient progress and modify care plans as needed
- Ensure compliance with HIPAA, agency policies, and regulatory requirements
- Participate in on-call rotation as assigned
- Perform other job-related duties as assigned
Requirements
- Active Registered Nurse (RN) license in the state of practice
- Current BLS/CPR Certification
- Valid Driver's License
- Reliable transportation and current automobile insurance
- BSN preferred
- Acute Care or Home Health experience preferred
- Strong assessment, communication, documentation, and care coordination skills
