RN Care Manager (Home Health)

Position Overview

The RN Manager – Home Health will create and sustain an environment where nurses and home health care teams deliver safe, efficient, and compassionate care. They will maintain a strong presence within the department, collaborate with physicians and multidisciplinary leaders, and ensure operational excellence across home health services.

Key Responsibilities

• Lead daily operations of the Home Health department

• Promote evidence-based clinical practice and patient safety initiatives

• Maintain compliance with regulatory, accreditation, and organizational standards

• Monitor quality metrics and implement performance improvement strategies

• Recruit, mentor, and retain top nursing talent

• Build strong leadership infrastructure through charge nurses and clinical champions

• Conduct performance evaluations, coaching, and professional development planning

• Foster a culture of collaboration, accountability, and open communication

• Manage staffing, scheduling, and resource allocation

• Support patient throughput and surgical efficiency initiatives

• Oversee department readiness, equipment safety, and emergency preparedness

• Address operational challenges and patient care concerns proactively

• Partner with surgeons, anesthesia providers, and interdisciplinary teams

• Participate in patient care rounds and care coordination efforts

• Drive improvements in patient, family, and physician satisfaction

• Ensure exceptional perioperative experiences and outcomes

Qualifications

• Bachelor’s Degree in Nursing or equivalent education and experience

• Current California Registered Nurse (RN) license

• Demonstrated leadership capabilities and sound clinical judgment

• Preferred Master’s Degree in Nursing or related field

• Previous Operating Room leadership experience

• Five years of clinical nursing experience

• Two years of acute care hospital nursing experience

• National nursing specialty certification

• BLS/CPR certification
