RN Care Manager - Home Health and Private Pay

About the role

Cheer Home Care is looking for an RN Care Case Manager who loves spending time with older adults and provides high-quality care in the home. The RN Care Case Manager collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, caregivers, and nurses. They work closely with the office team and have excellent communication and problem-solving skills. This position serves as the primary care manager, coordinating care among caregivers, RNs, therapists, and outside representatives. This role requires clinical expertise and experience working with private-pay clients and their families. Option to work one weekend day and have one weekday off.

Responsibilities

• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Provide constant feedback to clients, client representatives, caregivers, and nurses.
• Serve as the primary care manager, coordinating care among caregivers, RNs, therapists, and outside representatives.
• Work closely with the office team to ensure excellent communication and problem-solving.

Requirements

• Active California RN License.
• Minimum 2-3 years of home health nursing experience.
• Experience managing private-pay clients.
• Master in Nursing (preferred).
• Strong computer skills.
• Ability to multitask, drive, and take action.
• Strong organizational and communication skills.
• Pass Live Scan Fingerprint Clearance.
• Valid driver’s license and proof of insurance.
• Cell phone with internet access.
• Proof of negative TB results.
• Willing to work one weekend a month as back-up on call.

Pay

Salary: $120,000+ based on experience. Potential for team bonus.

Schedule

Option to work one weekend day and have one weekday off. Willing to work one weekend a month as back-up on call.
