RN - Care Manager

About the role

The Inpatient Registered Nurse (RN) Care Manager provides professional, comprehensive, patient centric care management services for at risk patients in an acute care environment. Accountabilities include assessment and planning, coordination of care, resource utilization management and/or review, discharge planning, documentation of interventions, regulatory compliance and patient advocacy.

Responsibilities

• Assessment and Planning: Conduct thorough assessments and develop individualized care plans for high-risk patients.
• Care Coordination: Coordinate interdisciplinary care teams to ensure seamless transitions of care.
• Resource Utilization Management: Review and optimize healthcare resource usage to ensure efficient and effective care delivery.
• Discharge Planning: Develop and implement discharge plans to facilitate safe transitions from hospital to home or community.
• Documentation: Maintain accurate and timely documentation of interventions, care activities, and patient progress.
• Regulatory Compliance: Ensure adherence to healthcare regulations and policies.
• Patient Advocacy: Advocate for patient rights and preferences to ensure their needs are met.

Requirements

• Associate's Degree in Nursing degree/diploma upon hire.
• Washington Registered Nurse License.
• 2 years acute care experience in a Medical Surgical or Inpatient setting.
• 1 year experience in care management or utilization review in any setting or upon hire successful completion of TIPS program or Case Management Orientation Program.

Qualifications

• Bachelor's Degree in Nursing or higher within 3 years of hire.
• National Certification in area of specialty.

• Excellent communication and interpersonal skills.
• Ability to manage multiple tasks and prioritize effectively.
• Proficiency in electronic health record systems.

Pay

The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. The base pay range is $47.69 - $94.06 per hour.
