Job Summary
Working under the direct supervision of the Clinical Services Director, the primary responsibility of the RN Care Manager is to provide direct and indirect nursing care to assigned patients by utilizing critical thinking skills to assist in the management of chronic diseases, clinical problem solving and decision making in collaboration with the IDT. The RN Care Manager works independently in the application of advanced nursing knowledge and skills relative to NCBON scope of practice, competency and experience. Participates in call rotation, performs home visits, end of life/palliative (aka Pathways), and nursing care per nursing protocol. Completes professional performance requirements annually.
Education/Experience
- Graduate of an accredited school of nursing.
- Preferred: Has practiced nursing in the last 3 years.
- Prefer experience in home care or long term care setting, and ideally, PACE experience.
- Two years of acute care management experience is preferred and/or 1 year of community based nursing experience.

Pay
$71,088 - $95,536 (commensurate with years of experience)
