About the role
In this pivotal role, you'll be instrumental in expanding our company and supporting our clients. Key responsibilities include extensive community outreach and providing clinical leadership.
Responsibilities
- Extensive community outreach
- Providing clinical leadership
- Strong relationships with local healthcare professionals and referral sources
- Outreach to healthcare professionals and referral sources to help grow patient census
- Handling various clinical duties such as initial assessments, weekly care management, coordinating with the Interdisciplinary Team (IDT), and overseeing caregiving team
- Oversight to all in-home caregivers
- Perform routine in-home supervisory visits per State Regulations
- Mentor and guide caregivers within their assigned region
- Provide education to caregivers, staff, clients, and other parties on clinical issues ranging from disease process to appropriate task performance
- Provide in-home nursing tasks such as wound care, medication administration, catheterizations, PIV/PICC dressing changes, blood draws, and more
- Conduct intake assessments and consultations
- Manage performance of caregivers
- Engage in community outreach and support services
- Provide care management services
Requirements
- Minimum educational requirement: Associate Degree in Nursing; Preferred: Bachelor’s or Master’s Degree in Nursing
- Must be a Registered Nurse (RN) in the state for which they are applying
- A minimum of two (2) years’ experience in geriatrics, home care, home health, and/or care management required; other professional experience may substitute
- Ability to work in a team environment
- Effective and professional communication with client committees, including family, physicians, attorneys, etc.
- Excellent written and verbal communication skills required
- PC literacy required
- Valid driver’s license and proof of current auto insurance required

Pay
The pay range for this role is $50,000 - $70,000 annually, depending on experience and qualifications.
Schedule
This is a hybrid position combining both in-home and office-based responsibilities.
