About the role
The RN Hospital Care Manager I provides comprehensive care management services to designated patients, utilizing clinical expertise for screenings, assessments, and evaluations. They develop and implement patient-centered care plans with shared goals and tailored interventions, collaborating with patients, families, healthcare providers, payers, and community partners to ensure efficient, high-quality, and patient-focused care across inpatient and outpatient settings.
Responsibilities
- Conduct comprehensive assessments to identify patient needs.
- Develop, adjust, and coordinate care plans addressing medical and social factors.
- Guide patient self-management through motivational interviewing and coaching.
- Provide education and advocacy to healthcare teams and patients, supporting self-care and informed decision-making.
- Collaborate with multidisciplinary teams to deliver high-quality care.
- Lead process improvement initiatives to enhance care transitions and policy adherence.
- Educate on goals of care and advanced care planning.
- Track key metrics, analyze trends, and ensure regulatory compliance.
Qualifications & Skills
- Current RN license in the practice state.
- Bachelor of Science in Nursing (BSN) or commitment to obtain within four years.
- Clinical nursing experience in hospital inpatient settings with knowledge of disease management and hospital processes.
- Skills in assessment, care planning, motivational interviewing, critical thinking, communication, and patient education.
- Proficiency in basic computer applications, with intermediate skills in Word and Excel.
Physical & Other Requirements
- Ability to see, read, and interpret information and labels.
- Effective verbal communication and listening skills.
- Manual dexterity for handling supplies and equipment.
- Mental stamina for high-stress situations and multitasking.
- For roles involving driving, ability to operate a vehicle safely.
