RN Care Manager

About the role

The RN Hospital Care Manager I provides comprehensive care management services to designated patients, utilizing clinical expertise for screenings, assessments, and evaluations. They develop and implement patient-centered care plans with shared goals and tailored interventions, collaborating with patients, families, healthcare providers, payers, and community partners to ensure efficient, high-quality, and patient-focused care across inpatient and outpatient settings.

Responsibilities

• Conduct comprehensive assessments to identify patient needs.
• Develop, adjust, and coordinate care plans addressing medical and social factors.
• Guide patient self-management through motivational interviewing and coaching.
• Provide education and advocacy to healthcare teams and patients, supporting self-care and informed decision-making.
• Collaborate with multidisciplinary teams to deliver high-quality care.
• Lead process improvement initiatives to enhance care transitions and policy adherence.
• Educate on goals of care and advanced care planning.
• Track key metrics, analyze trends, and ensure regulatory compliance.

Qualifications & Skills

• Current RN license in the practice state.
• Bachelor of Science in Nursing (BSN) or commitment to obtain within four years.
• Clinical nursing experience in hospital inpatient settings with knowledge of disease management and hospital processes.
• Skills in assessment, care planning, motivational interviewing, critical thinking, communication, and patient education.
• Proficiency in basic computer applications, with intermediate skills in Word and Excel.

• Effective verbal communication and listening skills.
• Manual dexterity for handling supplies and equipment.
• Mental stamina for high-stress situations and multitasking.
