Role Overview
The RN Hospital Care Manager I provides comprehensive care management for designated patients in inpatient and outpatient settings.
Responsibilities
- Conduct assessments and develop patient-centered care plans addressing medical and social needs.
- Support patient self-management through coaching and motivational interviewing.
- Educate and advocate for patients and healthcare teams to promote optimal care outcomes.
- Collaborate with multidisciplinary teams to ensure timely, high-quality care and smooth care transitions.
- Lead process improvement initiatives, including policy adherence and data analysis for compliance.
- Provide goals of care education to facilitate informed decision-making.
Qualifications & Skills
- Current RN license and demonstrated clinical inpatient experience.
- Bachelor of Science in Nursing (BSN); ongoing education requirements apply.
- Preferred: Case management certification, experience in discharge planning, utilization review, or value-based care.
- Skills in assessment, care planning, motivational interviewing, critical thinking, communication, and time management.
Physical & Other Requirements
- Ability to read, communicate effectively, manipulate equipment, handle high-stress situations, and, if applicable, drive a vehicle.
