RN Care Manager

About the Role

We are seeking an RN Care Manager to assess, develop, and coordinate personalized care plans for patients, addressing medical, social, and psychological needs. The role involves collaborating with healthcare providers, serving as a liaison between patients, families, and community resources, and managing patient transitions, referrals, and follow-ups.

Responsibilities

• Conduct patient assessments to identify medical, social, and psychological needs.
• Develop and implement personalized care plans in collaboration with healthcare providers.
• Serve as a liaison between patients, families, and community resources.
• Manage patient transitions, referrals, and follow-up care.
• Educate patients and families on care plans and health management.
• Maintain accurate and detailed patient records.

Requirements

• Bachelor's or Master's Degree in Nursing.
• Current RN license in Illinois.
• At least 2 years of clinical nursing experience, preferably in acute care.
• CPR/BLS certification.
