RN Care Manager - $15,000 Sign-On Bonus or Student Loan Assistance!

About the role

Reporting to the Supervisor of Care Management, this position will provide case management to our members, addressing medical, social, psychological, physical, and spiritual needs. You will develop, implement, and monitor care plans in collaboration with primary care providers, caregivers, and other team members to meet members' needs.

Requirements

• Associate’s Degree in Nursing (for experienced candidates) or BSN (for new graduates)
• Active NYS RN license
• CCM certification preferred
• 1 year of RN experience in managed care, home care, acute care, or nursing home preferred
