About the role

The RN Care Manager at Life Care Centers of America is responsible for coordinating patient care and services. This role promotes quality care through a collaborative process that coordinates, monitors, and evaluates services based on patient needs.

Responsibilities

  • Promote quality care using a collaborative process that coordinates, monitors, and evaluates services according to the needs of patients.
  • Work together with the patient and family, care team, payers, and external entities to ensure a safe transition from the facility to the next provider or care setting.
  • Generate written communication that is clear, concise, and well organized.
  • Organize tasks efficiently and manage time effectively.
  • Contribute to the organization's goals and objectives and support its strategic plans.
  • Provide expert knowledge in post-acute care settings.
  • Implement and interpret programs, policies, and procedures of the department.
  • Maintain professional relationships with all associates, vendors, and other providers of care.
  • Ensure confidentiality of proprietary and confidential information.
  • Adhere to company policies including harassment and compliance procedures.
  • Display integrity and professionalism by following Life Care’s Code of Conduct and completing required compliance training.

Requirements

  • Nursing diploma (associate’s or bachelor’s degree in nursing).
  • Currently licensed/registered in applicable State with an active RN license in good standing.
  • One (1) year of clinical experience in post-acute care setting preferred.
  • Prior case management, utilization review, and discharge planning experience preferred.
  • Certified Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) preferred.

  • Knowledgeable of practices and procedures as well as applicable laws, regulations, and guidelines.
  • Proficient in relevant computer systems for entering and retrieving data.
  • Ability to train and educate patients, families, associates, and other providers of care.
  • Strong interpersonal skills and excellent customer service.
  • Dependable attendance and ability to concentrate and use reasoning skills.
  • Ability to sit, stand, bend, lift, push, pull, stoop, walk, reach, and move intermittently during working hours.

Pay

$47-55 per hour, dependent on experience and qualifications.
