RN Care Management Coordinator

Primary Responsibilities

• Perform clinical assessments to develop a Plan of Care
• Oversight of discharge planning
• Provide education
• Establish overall coordination of care
• Identify members and those requiring referral to Clinical Pharmacy, Quality Management, or Wellness Coaching
• Collaborate with clinical team on complex member cases to provide consistent care

Requirements

• Bachelor’s degree in nursing
• RN licensure that is unrestricted in the state of Mississippi
• Intermediate proficiency utilizing software in the Microsoft Office Suite (Excel, Word, PowerPoint)
• Strong communication skills (oral/written)

Qualifications

• One year of clinical experience preferred
• Background in Hospital Case Management preferred
